Schedule an Appointment with a 1776 Care Coordinator
Once your information is received, we’ll schedule an appointment for a Care Coordinator to meet with you and your family. You can expect this to be solidified with us the same day, typically within 2 hours.
During the meeting with our Care Coordinator, you’ll be able to discuss and clarify all questions you may have for getting started with 1776 Senior Care, including potential start dates, rates, particular needs for your loved one and more. We’ll also have paperwork on hand for you to review to outline the home services agreement
Welcome To 1776 Senior Care!
From the moment of agreement, we’ll have you set up in your system with all vital information, including your senior’s Doctor and emergency contact as well as your billing information. We’ll also provide you a login for our Online Family Room, so you can start viewing activities or make requests from this portal right away, at your convenience.
30 days later, we’ll be sending you a client satisfaction survey. You can expect our Care Managers to make supervisory visits to your loved one’s household every 90 days, to ensure the highest standard of physical, emotional, social and spiritual needs are being consistently met.
